外科学(消化管外科)

外科学講座第一講座(消化器外科、乳腺・一般外科)は初代の故岩佐博教授が1974年3月に開講した。1983年に故玉熊正悦教授に引き継がれ、1996年から望月英隆教授が主宰した。2006年4月に外科学第一講座と外科学第二講座(呼吸器外科、心臓血管外科、上部消化器外科)が合併し、外科学講座となり大講座制に移行した。
2008年4月の防衛医科大学校組織改組に伴い、外科学講座には外科1(消化管外科)、外科2(心臓血管外科、呼吸器外科)、外科3(肝胆膵外科、乳腺外科、小児外科)の臓器別区分が新たに設定され、望月英隆教授の防衛医科大学校病院長就任に伴い、その後任として長谷和生が外科1の教授に就任した。
2015年4月の長谷和生教授の防衛医科大学校病院長就任に伴い、外科学講座教授の公募、教授選考が行われ、2016年4月に上野秀樹が第5代教授に就任した。現在の本講座の教育・診療・研究の指導は、下部消化管外科を上野秀樹(外科第1部門長 兼 下部消化管科長)、神藤英二、梶原由規、望月早月、岡本耕一が、上部消化管外科を辻本広紀(副院長(医療安全担当) 兼 上部消化管科長)、堀口寛之が担当し、上野秀樹教授がふたつの科を統括している。

消化器・一般外科疾患の診療と教育、研究を系統的かつ効率的に行うため、学生、研修医、専門研修医、研究科学生の臨床・研究教育は、岸庸二教授が主宰する外科3と合同で行っている。外科1・3の医局会を毎日開催し、臨床カンファレンス、抄読会、学会予演会についても合同で行っている。また外科2(田口眞一教授主宰)を含む外科学講座全体での教官会議を毎月開催し、教育をはじめとする講座運営に関する意見交換を行っている。本校卒業生の日本外科学会専門医の円滑な取得は教官会議開催の大きな目的のひとつである。

1.学生
当科では消化管疾患の外科治療と外科学総論の教育を担当している。
第3学年学生の教育:総論は外科的診察・消毒・滅菌・基本的手術手技・損傷と創傷治癒・手術侵襲と生体反応並びに術前術後管理・輸液と栄養管理・ショック・炎症・免疫を、各論は消化管・急性腹症を教育している。
第5学年学生の教育:4週間ごとにラウンドしてくる臨床実習の学生を4つのグループに分け、教育指導医(スタッフ・研究科学生)、指導医(専門研修医)、初任実務研修医から構成される臨床チームに組み込み、チームと共に行動を共にするCCS方式で実施している。その内容は手術室実習、病棟患者管理実習、内視鏡検査の見学と画像読影、教授回診や術前術後症例検討会への参加、動物実習、クルズスなどで、密度の高い教育を行っている。
第5学年、第6学年に対する臨床講義では、臨床実習中の学生が自ら受け持った患者のプレゼンテーションを行う学生参加型の教育を行っている。

2.初任実務研修医
初任実務研修医に対しては、消化器一般外科疾患の知識、外科的基本手術手技、周術期のおける患者全身管理技術、terminal careを中心として教育している。受け持ち患者数は研修医1名あたり常に10名以上であり、特にICU管理が必要な患者を受け持つと極めて濃厚な研修となる。病院内で最多の病床数を保有する消化管外科の研修医は、疾患の特性からも多忙な日々を過ごすことになるが、当講座は伝統的に明るい雰囲気を有しており、指導体制も明確に確立されている臨床チームの一員として臨床に携わることより、研修終了時には全診療科の中でも最も楽しく有意義な研修であったと評価する研修医が多い。

3.専門研修医
外科専門医を取得し得る知識と技量を身につけ、必要な手術症例数を経験させることを目標として教育しているが、現在までその目標は確実に達成されている。また更に難易度が極めて高い消化器外科専門医の取得の基盤を作ることも目標としている。
近年専門研修医として本校に戻ってくる卒業生が増加しているため、3年の専門研修医期間の内の一定期間を学外で研修する部外研修システムを設け、部外研修病院の充実も図っている。一般消化器疾患の症例を数多く経験することを目的に近隣の病院(埼玉医大総合医療センター、所沢明生病院、所沢中央病院、所沢肛門病院等)での研修や、また最先端の癌治療の習得を目標として都内病院(がん研有明病院、虎の門病院、帝京大学医学部付属病院、帝京大学付属溝口病院、国際医療研究センター病院等)での研修を行う制度を設けている。この制度により専門研修医にとっては外科医としての幅が広がるのみならず、他大学の同世代医師と一緒に研修することにより、適切な刺激を受けるメリットがあると考えている。

4.研究科学生
本講座には研究に関して自由闊達な学風があり、これが学術領域の多様性及びオリジナリティーの創出に貢献してきた。研究科学生は下部消化管外科グループと上部消化管外科グループのいずれかを選択し、教授とグループの教官による研究指導を受ける。実臨床に還元する研究をおこなうことを徹底して指導しており、その研究成果の一部は、実際に治療ガイドライン改定の礎となっている。国際的な感覚を身に着けることは外科医としての成長の大きな糧になると考えており、海外留学を推奨している。これまで多くの研究科学生が、米国、英国、カナダにおける有名研究施設のresearch fellowとして活動している。
研究科学生には学位取得に向けての指導のみでなく、臨床面での成長ができる環境も提供することも配慮しており、教官の監督下で手術指導の経験も積ませている。また、多施設共同研究に積極的に関わるように指導を行っている。

下部消化管外科

下部消化管外科グループが携わった多くの研究のうち、「大腸癌症例における予後予測に重要な組織学的パラメータの探索」はグループ創設期から現在に至るまで、臨床研究の主たる柱である。特に教室より提唱した簇出(budding)の定義、評価基準は国内で注目され、大腸癌治療ガイドラインや大腸癌取扱い規約に掲載されるに至った。近年では国際的にも注目され、平成28年4月にスイスのBernで開催されたbudding国際コンセンサス会議にて世界の評価基準として採用されることが決定された。現在簇出に関わる分子生物学的なアプローチの一環として、DNAマイクロアレイのデータをもとにbuddingと関連する遺伝子群を同定するとともに、遺伝子発現指標(budding signature)の構築を進めている。

簇出以外にも、EXや神経侵襲、低分化胞巣、desmoplastic reaction等、癌浸潤先進部の外科病理学に関する本グループからの報告は外科、病理学、腫瘍学の主要なjournalに掲載されており、その一部はNCCNガイドラインや海外の成書に複数引用されるなど、この方面の研究は着実に国際的にも認められつつある。癌浸潤先進部に特異的な腫瘍形態の分子生物学的背景の解明は、現在複数の医局員が研究テーマとしており、大腸癌より分離培養した癌関連線維芽細胞や大腸癌オルガノイドを活用した基礎研究を積極的に行っている。
一方、多施設臨床研究の重要性に関する意識も高い。外科手術に関する研究に関して、現在結腸癌における切離腸管長を確立することを目的とする国内の多施設研究を主導しており、英国、ドイツ、韓国、ロシア等との国際共同研究の事務局としての業務も担っている。また平成28年に大腸癌研究会にてpT1癌に関する日英共同研究が開始され、本グループが研究責任者を務めている。組織学的研究に関連する分野での近年の新たなる展開は、進行直腸癌症例における術前化学放射線療法あるいは術前化学療法の効果予測指標に関する研究である。この分野の研究は学外からも注目され、現在多施設研究を推進中である。大腸癌研究会やJCOG(日本臨床腫瘍グループ)で行われている多施設研究にも積極的に研究分担施設として参加しており、本邦発のエビデンスの構築に貢献している。

上部消化管外科

グループの研究は、当講座の伝統である「外科侵襲学」に関する研究を継承して、「研究は臨床に根付いたものでなければならない」という信念のもと、悪性腫瘍の進展・転移のメカニズムとその制御、低侵襲手術の客観的評価、侵襲時あるいは重症感染症の病態解明などが主たる研究の目的となっている。胃に関しては、胃癌における見張りリンパ節理論の検証、腹腔鏡下胃切除の有用性や新しい術式の開発といった実臨床に直結する課題を始め、癌幹細胞理論からみた胃癌制御の可能性やc–Met癌原遺伝子制御による基礎的検討など多岐にわたる。Sentinel node navigation surgery研究会や日本臨床腫瘍グループ主催の前向き臨床試験に積極的に参加し、世界で類を見ない新しい術式の開発や、至適リンパ節郭清範囲の決定などのevidence構築に向けてグループ一丸となって活躍している。食道に関しては、近年導入した胸腔鏡下食道切除の低侵襲性を全身性炎症反応症候群診断項目から客観的に示した。一方、「外科侵襲学」と「腫瘍学」の接点として、ヘリコバクターピロリが有するエンドトキシンが胃癌の増殖に大きく関与し、また重症感染症や過大侵襲時に組織修復にとって重要な役割を有するHepatocyte growth factorが、胃癌や食道癌細胞上のc–Metと結合することで腫瘍進展に大きく関与することを示すなど、「侵襲腫瘍学」という新たな学問の概念を内外に示した。また最近ではナノ医学を駆使したドラッグデリバリーシステムを利用して、光線力学療法による悪性腫瘍やヘリコバクターピロリ菌の除菌への臨床応用に向けた基礎的研究を行っている。
